Getting out
Rides to the doctor, to church and to the store.

- Medical and dental appointments
- Pharmacy and grocery runs
- Faith services and social visits
- Staying with the client during the visit

Care coordination
One person tracks the plan. You do not have to.
What is included
- A written plan reviewed as needs change
- Coordination with doctors and case managers
- Regular updates to the family
- Help understanding coverage
